Students can request accommodations relating to academics, housing, or facilities access using this form. Students should request accommodations as soon as possible prior to the start of classes. We recommend submitting accommodation requests for the Fall Semester no later than August 1, and January 1 for the Spring Semester.
ADS' review process for any accommodation request may take up to two business weeks (sometimes longer during peak seasons) to complete. Incomplete requests may result in further delayed action.
- Complete the online Accommodations Request form.
- If you have previously requested accommodations, please login here to ensure your information is up to date and request letters for the current semester .
- Upload Supporting Documentation from your relevant provider to the online form.
- ADS staff will schedule an Intake Meeting with you to determine reasonable and appropriate accommodations based on impact of disability, previous history of accommodations, and documentation. We will also discuss how to implement potential accommodations.
- Receive an Eligibility Letter outlining approved accommodations and/or next steps via your MICA email.
- Request Faculty Accommodation Memos in AIM each semester and discuss implementation with individual faculty, with ADS support as needed. Please note that accommodations are not in effect until Faculty Memos are sent out.
- Complete any relevant additional steps as outlined in your Eligibility Letter in order to utilize specific accommodations (e.g. Notetaker Request Form, Audio Recording Contract). Some accommodations may require one additional step in order to be utilized, so make sure you read your Eligibility Letter carefully.
Important Notes About Disability Accommodations
- Students can register for accommodations at any time during the semester, but accommodations are not retroactive.
- Academic accommodations take effect when the faculty member receives a notice letter.
- Mid-semester housing accommodations may require a student to be put on a waiting list until an appropriate unit can be made available.
- Accommodations are determined based on the interactive process that occurs during the Intake Meeting.
- The intent of accommodations is to provide equal access to learning or housing.
